Questionnaires and Surveys Mailed

The annual Ministerial Questionnaires and Church Surveys have been mailed to all Ordained Ministers, Licensed Ministers, Commissioned Christian Workers, Conference Care members, Lay Preachers and member churches.  Members and church leaders can return a hard copy of the questionnaire or survey via mail to the home office or fill out the form online at: https://www.ccccusa.com/resources/applications-forms-2/online-ministerial-questionnaire/ or https://www.ccccusa.com/resources/applications-forms-2/annual-church-survey/ . 

We encourage our members to complete the questionnaire as soon as possible. Your assistance to complete this information and turn it in to the Home Office by December 15, 2020 is greatly appreciated.
